Job summary
To assist with the Subject Access Team in the delivery of a high quality service by:
- Providing a high standard of processing requests for information
- Ensuring that legislation is adhered to at all times
- Ensuring that requests for information are processed within the legal timeframe
Main duties of the job
Providing a professional end to end service for the processing of information requests.
- Recording all requests on an electronic system
- Retrieving all records to satisfy the request
- Redaction of records within the legislation guidelines
- Liaising with senior clinical staff for authorisation to release records
- Communicating with requesters to ensure they are informed of updates
- Quality checking information being released to requesters

Job responsibilities
- To be responsible for the processing of subject access/access to health record requests, ensuring that confidentiality is maintained at all times.
- To be responsible for managing the timescales for subject access requests, ensuring they are completed within the allocated time and avoid breaches.
- To be responsible for maintaining records of subject access/access to health record requests.
- To be a contact for enquiries relating to subject access.
- To fully participate in the supervision and appraisal process.
- Maintaining the Team Share point site, uploading documents and updating information when requested.
- To escalate any issues or potential issues to Line Manager
- To work collaboratively with key colleagues across the Trust
- To communicate sensitive information
- To communicate with clients with regards to subject access requests
- To coordinate complex extraction of data and information from a number of different systems.
- Quality audits of SAR responses sent.
- To ensure all analytical data is presented appropriately when required.
- To assist in the preparation of reports and the monitoring of action plans for subject access
- To access all EPR systems and track paper patient records as per Trust policy.
- To adhere to the Code of Conduct for Administrative staff.
- To act as a central point of contact for the Trust for Subject Access
- To maintain staff records ensuring recording in this regard is up to date and entered timely.
