Job summary
This is an opportunity to join a dynamic and forward-thinking Urgent Care Mental Health Team.
In line with the NHS long Term Plan we are developing a new model of Urgent Care Services as an Alternative to Hospital Admission within Central Manchester and require a dynamic night time practitioner.
The post holder will be the sole Home Based Treatment Team Central's night practitioner who will interface closely with the MHLT and Crisis Point to implement and develop access to and delivery of Urgent Care services in Central Manchester.
The post holder will be co located at the MRI with MHLT. This is a developing role with the potential of being based at Crisis Point in Central Manchester in the future to further develop mental health crisis pathways for Central Manchester patients.
The post holder will be a designated night time practitioner for Home Based Treatment Team Central.
Main duties of the job
- The post holder will work as part of the urgent care team and will be co located with the MHLT at the MRI in A & E Liaison. There is a future plan for the post holder to be located at Crisis Point as GMMH urgent care services develop.
- The post holder will organise the HBT's following days work so that the day team are able deliver safe, timely and effective care.
- The post holder will be expected to work regular night shifts that include some weekends.
- The post holder will follow safeguarding policies and protocols and share relevant information with other agencies as appropriate.
- The post holder will be aware of and act within the requirements of the Mental Health Act (1983), and other relevant legislation at all times,
- The post holder will participate in activities as directed by senior staff.
- The post holder will undertake collaborative care planning with service users and their carers.
- The post holder will undertake risk assessments and recording of risk in line with Trust Policy.
- The post holder will maintain their knowledge of evidence-based home treatment interventions and deliver these interventions in line with service user's planned care.
- The post holder will complete Home Based Treatment Team assessments both in A&E and in community settings.
- Post holder will undertake gate keeping assessments to ensure that service users are treated in the least restrictive environment.
