Job summary
Under the line management and clinical supervision from within the CMHT, the post holder will maintain and co-ordinate timely and appropriate interventions through mental health nursing, assessment for a caseload of service users living in the community within the framework of CPA, and in line with relevant clinical pathways (Functional or Organic) as outlined within the CMHT operational procedure. This includes respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
Main duties of the job
To act as care coordinator for case load of clients with complex needs in line with CPA policy.
Work to best practice ensuring sound risk assessment and management.
Work as part of the MDT.
Act as an advocate for your patients.
Participate in duty rota including weekends and bank holidays on a rota system.
Support and direct student nurses and support junior staff.
Work autonomously, have excellent time management skills.
Engage in line management supervision.
Work as part of integrated health and social care team, working under The Care Act (2014).
