Job summary
Due to staff member retirement within the team an opportunity has arisen to work as a Community Mental Health Nurse in Older Adults Community Mental Health Team in Salford. This is a progression post to become a CPN Band 6 within 12 months. Subject to achievement of criteria.
We are looking to recruit a band 5 Community Mental Health who has a passion for working with older people to join our developing, supportive, and well led multidisciplinary team.
Our team work collaboratively with service users and their families in a recovery-orientated model, promoting independence, choice, wellbeing, and social inclusion with an emphasis on supporting individuals to achieve their hopes and aspirations. Working with both functional and organic mental health difficulties.
Our team promotes professional identity and development with many CPD sessions both in house and external speakers on a monthly basis.
As this is a progression post there is an expectation that the candidate will progress to a band 6 CPN post following successful induction and training period.
Main duties of the job
The Community Mental Health Nurse will work as part of the multi-disciplinary team. They will manage a clinical caseload of older adults with mental health difficulties, offering assessment and intervention.
As this is a progression post the successful candidate will be working towards:
- Act as a Care Coordinator and fulfil the requirements of this role.
- Support a caseload of older adults with mental health problems, using evidence-based, client-centered principles to assess, plan, implement, evaluate, and record interventions.
- Provide support and advice to carers.
- Participate in the duty rota.
- Provide a nursing perspective within the multi-disciplinary team.
- Maintain nursing standards and contribute to the development of nursing practice in the team.
- Contribute within the wider MDT in the development of clear care pathways regarding support, care and treatment of older adults with mental health problems.
- Support student placements.
- Work weekends and bank holidays on a rota basis.
