Job summary
We are looking for a social worker to start with our Living Well Team. The post holder will undertake a key worker role, alongside an assessment and formulation role within the team. The Living Well Service will enable timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team will be multidisciplinary and will also include third sector organizations. The areas that this team will cover (second Living Well Team) are Turton, Halliwell, Horwich, Westhoughton & Chorley Roads.
You will be expected to travel around a large footprint.
Main duties of the job
You will need to have experience of triaging and signposting referrals as well as being able to liaise with multitude of agencies and professionals. Experience and knowledge of community services and third sector organizations in Bolton will be beneficial for this role.
Bolton Living Well Service will be compassionately led, trauma informed, and recovery focused. We will also be developing peer mentor opportunities for people who have been through the service. CPD opportunities are available and personal interests/skills will be supported to develop and grow.

To establish and maintain robust communication networks with clients, carers, and other health care workers and numerous external agencies.
To establish and maintain good working relationships with GPs, and other stakeholders.
To promote awareness of the role of Living Well within local service area/team, GP practices and the wider community.
Establish strong links with community mental health teams by continually building on interface and relationships to ensure smooth transitions for service users.
