Job summary

An exciting opportunity has arisen for an RMN to apply for the post of a Band 5 CPN in our Memory Assessment and Treatment service based at Woodlands Hospital in Little Hulton.

We are looking for a highly motivated individual who has an interest in the treatment of dementia and support following a diagnosis to work within our large multidisciplinary team in monitoring dementia patients within the community. You will require sound communication skills and the ability to participate in making clinical decisions with regard to treatment both with clients and their families and as part of a multi - disciplinary team. Our Client group is predominantly older adults but involves work with clients diagnosed with young onset dementia and a good knowledge of the needs of dementia patients is required to enable timely and effective interventions. Good community risk assessment and management skills are essential. Experience working in a community setting is preferable both in patients' homes and other community venues and being able to travel the GMMH footprint in order to undertake this role. The hours of work are predominantly Monday to Friday 9am - 5pm.

Main duties of the job

Band 5 nurses are based at Woodlands Hospital and complete a mixture of reviews, including home visits, clinic appointments and phone reviews.

The main duties of a band 5 in MATS are to oversee the commencement and titrations of dementia treatments and then once established on treatment to complete 6 monthly reviews of the medication.

The role includes liaison with other members of the team particularly medics in order to make changes to prescriptions.

Band 5 nurses are expected to support students and supervise junior colleagues.

Under the line management and clinical supervision from within the MATS, the post holder will:

  • Participate in the mental health nursing assessment, medication titration, monitoring and review of service users with diagnosed cognitive impairment or dementia, as defined in the NICE guidance.
  • Maintain and co-ordinate timely and appropriate interventions for an allocated caseload of service users of any age living in the community within the standard care CPA framework and operate in line with the relevant dementia assessment clinical pathways as outlined within the MATS Standard Operational Procedure (SOP).

This includes respecting individuality, values, culture and religious diversity of service users and colleagues and thus contributing to the provision of a high-quality service sensitive to these needs.
