Job summary

We are looking for for an experienced, well organised, Registered Healthcare professional to cover the Maternity leave of our current Discharge Hub Coordinator.

The post holder will be an experienced clinician or Social Work practitioner working within the mental health, learning disability and autism team (MHLDA) and will sit within the Birmingham and Solihull MH&LDA Provider Collaborative.

The post holder will be expected to work alongside senior clinical leads and colleagues to provide oversight and co-ordination of the inpatient cohort including admission avoidance strategies and facilitating the discharging of clients.

The post holder will require specialist clinical knowledge of the health needs of people with learning disabilities, autism and/ or mental health as well as local and national priorities across health services and the impact of these on people with learning disabilities and autism with or without mental health needs.

An understanding of health and social care systems within the Birmingham and Solihull locality would also be beneficial. The successful candidates will also have highly developed reporting and analysis skills and be confident with having outcome focussed and collaborative conversations with a variety of stakeholders to promote patient outcomes and system challenge, working with both NHS and non NHS providers and Local Authorities.

Care reviews are being undertaken both virtually and in person, so an ability to travel is essential.

Main duties of the job

To work with clinical and social care staff in developing appropriate changes in the discharge pathway to support developments as part of the LDA Strategy

Take the role of lead, chair, organiser, coordinator or facilitator at meetings relevant to the role including system wide discharge meetings.

To be responsible for ensuring that a high standard of patient care is delivered within the service

To monitor & escalate any delayed discharges across providers

To ensure that activity against the agreed performance management indicators for the service area is measured & communicated to service commissioners & other interested parties.

To work jointly with all BSOL community providers to oversee progress against 12 point discharge check list to enable timely discharges.

Provide & receive highly complex, sensitive & contentious information, presenting information.

To identify escalation of any concerns identified via safe & well checks , visits & to deal with any resulting challenging/ complex situations.

Support Court of Protection work as required.

To be responsible for ensuring that effective communication systems are in place for all staff working within the service across the Integrated Care system.

To be responsible for ensuring that a high standard of patient care is delivered within the service.

To proactively seek alternatives to admission for referrals and inpatients in collaboration with the care coordinators.
