Job summary

An exciting opportunity has arisen for a hub manager for the BSMHFT Community Mental Health Team for Homeless People, Homeless Health Exchange Primary Care Team and the NHSE funded Mental Health Outreach Team for Rough Sleepers.

All teams operate city wide to meet the needs of Homeless adults with enduring mental health problems and often substance misuse issues who require significant engagement due to their chaotic lifestyle. The teams work in partnership with other homeless commissioned statutory and third sector organisations for accommodation, housing and tenancy support, physical health care and substance use services.

The CMHT is a small team whose CPN's work with a consultant psychiatrist to provide secondary care level mental health interventions with a focus on stabilisation of lifestyle, recovery and moving out of homelessness.

The team working with rough sleepers has an engagement, assessment, referral and signposting case management role to manage mental health issues, in order to allow other agencies interventions to support the individual off the streets and start their journey away from street homelessness into a less chaotic and damaging lifestyle.

The Homeless Health Exchange Primary Care Team is a GP practice which support those who are homeless and work in partnership with Strensham Road GP Practice.

An ability to make relationships, provide sustained support to non-engaged individuals and work in sometimes difficult conditions are required.

Main duties of the job

- The post holder will provide effective and efficient management of delegated resources within a defined clinical area while ensuring care delivery meets and addresses the issues of spirituality, equality, diversity and rights in accordance with good practice and legislation.

- To lead in the management of all clinical risks in their designated area, including serious and critical incident management, periodic review and analysis of service development, service performance and service needs/overview.

- To conduct and oversee audit of core activity and relevant aspects of services provided.

- The post holder will lead in the assessment, planning, implementation and evaluation of care across the clinical services, and ensure that staff have a working knowledge of local, national policies and legislation which govern current service provision.

- Provide advanced clinical skills, leading on practice issues and providing leadership and management to teams.

- In case of untoward incidents, co-ordinate reporting including contact and liaise with appropriate agencies and personnel.

- Work with partner agencies (statutory and third sector) to provide effective strategies to proactively manage the care of individuals with complex needs.
