Are you a dynamic, innovative, and enthusiastic Advanced Clinical Practitioner with a passion for supporting individuals with memory difficulties? Do you want to be part of a pioneering initiative that is reshaping Dementia and Frailty services across Birmingham?

We are excited to offer a unique opportunity to join a newly funded project under theBirmingham Fairer Future Fund, aimed at tackling health inequalities through culturally sensitive and collaborative care pathways. This role will be central to theco-design and delivery of new assessment models, working closely with system partners and community stakeholders to improve awareness and access--especially for under-represented groups.

Essential Criteria:

  • Registered healthcare professional with ACP qualification.
  • Experience in dementia care or memory services.

  • Excellent communication and stakeholder engagement skills.
  • Ability to work autonomously and collaboratively in a multi-agency environment.
