Job summary
To deliver end-to-end case management for BSOL patients in out-of-area independent rehabilitation placements, providing continuous review of clinical care, care-plan quality and risk assessments, and securing timely repatriation or community transitions. This role ensures that each patient's discharge pathway is optimised, that barriers are identified and addressed early, and that all movements in and out of area are captured, analysed and reported to demonstrate efficiencies and savings .
Main duties of the job
The Discharge Manager will manage a caseload of approximately 25-30 patients, coordinating reviews and attending multidisciplinary team meetings to monitor care delivery and progress. They will update and maintain accurate patient records on RiO, flag any discharge challenges and drive solutions in partnership with CMHT, EIS, AOT, S2R and ICRT colleagues. By forging and sustaining relationships with out-of-area providers and local services, they will expedite step-down placements into local Trust level 1 or 2 rehabilitation beds or community rehabilitation. Working closely with informatics and project teams, they will capture, analyse and present data on bed-days, length of stay and cost-savings, using dashboards to inform continuous service improvement.
