Job summary

The post holder will work in the function of Liaison Mental Health Nurse between Birmingham & Solihull Mental Health Foundation Trust and the Acute hospital, as part of the Psychiatric Liaison Team Assessment. To be responsible for the assessment and coordination of care needs, planning and implementation of evidence-based care to service users and their carers within the acute hospital. To provide professional advice and support to other agencies and members of the multidisciplinary team. As part of a team, you will deliver a proactive clinical service focusing on improving healthy wellbeing whilst also reducing the burden on acute hospital facilities through driving timely discharge and utilising integrated community pathways to reduce readmission.

Main duties of the job

:Will work as a practitioner within the acute hospital whilst liaising with other professionals.

:Will work as a generic psychiatric Liaison Specialist Pratitioner Nurse.

:Responsible for the assessments, planning implementation and evaluation of nursing care within agreed parameters and have a working knowledge of local, national policies and legislation which govern current service provisions and audit.

:Will be responsible for ensuring effective communication pathways are maintained.

:Will be responsible for ensuring the promotion of equality, diversity and rights in accordance with good practice and legislation.

:Take responsibility for identifying and contributing to the training needs necessary in providing quality evidence-based care as apporpiate.

Support senior colleagues in the effective of assessments and delivery of the Psycchiatric Liaison clinical service.
