Job summary

An exciting opportunity has arisen for Band 6 Primary Care Practitioners to join our Primary Care Mental Health team in Solihull, working within our local Primary Care Networks (PCNs).As part of the Long Term Plan, our community services in Birmingham and Solihull are undergoing huge transformation. Our new mental health teams are working within GP surgeries, with the aim of making services more accessible, more timely, and embedded within our communities. We are working closely with many other partners and voluntary services, to ensure a truly holistic approach to care. Main duties of the job

You will be responsible for providing mental health triage, assessment and outcome based interventions to meet service users needs.

You will be responsible for promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.

You will support and advise colleagues within the PCN and across local neighbourhood multi-agency teams.

Crucial to this role will be your ability to ensure robust communication across mental health providers, VCSE organisations and within primary care.
