Purpose of Role:
To provide dedicated, continuous supervision and support to a patient identified as requiring 1-to-1 security due to risks such as aggression, absconding, self-harm, or vulnerability. The role ensures the safety of the patient, staff, and others within the hospital environment.
Main duties of the job
- Continuous Supervision
- Maintain constant observation of the assigned patient to ensure safety and prevent incidents such as absconding, self-harm, or aggression.
Risk Prevention
- Identify and respond to potential risks or triggers.
- Intervene early to de-escalate situations using approved techniques.
Patient Escort
- Safely accompany the patient during movements within the hospital (e.g., to appointments, wards, or outdoor areas), ensuring secure and controlled transitions.
Incident Management
- Respond promptly to any incidents involving the patient.
- Support clinical staff during emergency situations or behavioural escalations.
Communication
- Maintain clear and professional communication with nursing and medical staff.
- Report any changes in patient behaviour, mood, or condition.
Documentation
- Accurately record observations, incidents, and interventions in line with hospital procedures.
- Complete handover notes for continuity of care.
Safeguarding & Confidentiality
- Adhere to safeguarding protocols and maintain patient confidentiality at all times.
Professional Conduct
- Maintain a calm, respectful, and non-judgmental approach.

- Ensure secure transitions between departments while maintaining dignity and privacy.
3. Incident Response - Act as first responder to behavioral incidents involving the patient.
- Support clinical staff in managing challenging behavior using approved de-escalation and restraint techniques if necessary.
- Complete incident reports accurately and promptly.
4. Communication & Collaboration - Liaise regularly with nursing, medical, and security teams regarding patient status and risk factors.
- Participate in handovers and multidisciplinary briefings.
- Report any changes in patient behavior, mood, or condition.
