An exciting opportunity has arisen for an enthusiastic, motivated and committed Community Practitioner to join the current Regulated Care Team within Intensive Home Support on a fixed term basis.
The role Community Practitioner is to be a clinical asset within the regulated care setting, providing direct care, education, and training to patients and care providers. Using advance care planning working with MDT & care providers this role will be responsible for the Holistic Care of patients within Regulated Care Settings.
The post holder will review hospital discharges back to regulated care settings and complete their holistic health assessment to ensure patient safety, this includes medication reviews and frailty. Some of these patients will already have assessments completed, utilising the MDT and own knowledge ensure that the appropriate care is being provided.
