Job summary

We are looking to recruit an enthusiastic to lead and develop the delivery of SLT in care homes across Pennine Lancs. This post is suitable for anyone with current band 7 SLT experience or an SLT wanting to start their band 7 journey. An established and valued pathway is in place which utilises and benefits from a hybrid model of service delivery, which means that some home working would be possible.

The job focus is to work alongside community SLT colleague to continue the design and delivery of new models of care working in partnership with care homes to understand how we meet their needs and how the service can best allocate resources.

The post holder will be part of the Pennine Lancs community team and will work closely with the Community SLT Clinical Team Leader to ensure any new ways of working are embedded and sustainable.

The Therapies directorate is committed to delivering integrated clinical pathways, with a range of transformation initiatives, including promotion of the acute-community interface and safe timely discharge.

Main duties of the job

Provide operational management and clinical leadership as defined within the job plan to the designated clinical teams.

Act as the Clinical Lead for Care Homes providing a high level of clinical expertise with regard to highly specialist assessment skills and interactions.

Provide highly specialist advice/intervention to support the assessment, management and discharge planning of patients within care homes.

Provide highly specialist clinical care to residents in the Care Home sector with a strong focus on preventable and avoidable admission.

Provide teaching and highly specialist support in the management of complex conditions, educating other healthcare professionals and wider stakeholders.

Contribute to the delivery of a highly effective, safe and personal service, delivered around the needs of individual patients/carers as described by agreed goals/outcome measures.

Undertake a team leader role in the planning, co-ordinating, delivering and evaluation of the service provided by the clinical team.

Be part of the management structure and contribute toward the strategic and clinical development of the service.

Be responsible for deputising for the Clinical and Operational Service leads.

Undertake project and service improvement work as appropriate.

The service is well equipped and is supported by a team of administrators, and Therapy Assistant Practitioners. Regular team meetings, an annual SLT Showcase of best practice and SLT, Stroke and Neuro Improvement Showcase.Continual improvement is embedded in our daily work and service improvement initiatives.We are committed to continuing professional development and this is underpinned by supervision, regular line management and an annual appraisal system.
