The Specialist Medicines Optimisation in Carehomes Team (SMOCH) provides a pharmacist led community-based service to nursing and residential homes. The team is fully integrated in community service provision. Working with multidisciplinary community teams is part of our daily work with the objective of supporting medicines optimisation and improving patient outcomes.

Main duties of the job

The position will involve daily travel between nursing and residential homes across East Lancashire. The successful candidate must have the use of their own vehicle for daily business use. When not carrying visits, the team work remotely and have access to community based sites.

It is essential that you possess excellent communication skills and are able to communicate and connect at all levels, often with people who may be vulnerable or have diverse needs. You will be self-motivated with the passion to deliver a patient focussed service, providing quality outcomes and experience for the patient. We support integrated working across both primary and secondary care on a multidisciplinary level to support prescribing and safe management of medicines. We are able to provide support for patients on discharge from hospital where there are changes to medicines, or complex medicines support needs have been identified during their stay.

We support our nursing and residential homes with medicine management issues and provide resources and training to help them comply with CQC requirements.

The service operating hours are Monday to Friday, within the hours of 08:00 and 17:00, although we would like to support the successful candidate with any needs for flexibility.

  • Registration with the General Pharmaceutical Council (GPhC)
  • City and Guilds, B/Tech in Pharmaceutical Science, NVQ Level 3 or equivalent
  • Evidence and commitment to life-long learning

Experience

Essential

  • Recent General Clinical Pharmacy Activity
  • Substantial post qualification experience in hospital and/or community

Desirable

  • Knowledge of both primary and secondary care pharmaceutical services
  • Previous experience of providing prescribing support in primary care

Work Related Knowledge and Skills

Essential

  • Knowledge of medicine uses and their actions
  • Multi-disciplinary working.
  • Basic IT literacy i.e. use of e-mail, the internet and word processing

Desirable

  • Experience of using primary care clinical systems
