Job summary

To act as a highly specialised podiatric clinical resource and be responsible for the professional/operational design, provide effective, efficient delivery of service and maintenance of high-quality care within this specific and high-risk field of the diabetic foot; for the Multi-Disciplinary Diabetes Foot Team (MDFT) Service.

To work directly with the Extended Scope Practitioner (ESP)/Integrated Diabetes Service Manager (IDSM in designing and effecting appropriate evidence based, outcome driven patient pathways within the MDFT Service and lead in working with partner organisations and stakeholders in providing seamless integration of such pathways for the treatment and care of patients with, particularly high risk, diabetic foot complications but also the diabetic foot in general.

To lead and implement a regular review of outcome measures across the service to ensure the service is effective.

To carry a highly complex and high-risk clinical caseload.

To lead clinically and professionally the MDFT.

To lead/facilitate and partake in research, audit and quality improvement/assurance programmes in area of own expertise.

To act as an operational support to the ESP and IDSM when required.

Main duties of the job

Works directly with the ESP/IDSM in designing/planning strategic development and design of sub-specialist podiatric field of the patient with a diabetes foot. Design relevant high level internal pathways for efficient treatment and care of high risk and complex diabetic foot complications by self and with the MDFT to generate pathways for general/routine care and prevention of complications within the wider podiatry arena and seamlessness between the three areas (ELHT and BwD) Under direction of ESP/IDSM, leads implementation of above pathways to deliver improving quality and service delivery for the future.

Leads work with partner organisations/stakeholders to effect appropriate external diabetic foot pathways, synonymous with and parallel to (where appropriate) internal pathways; to provide seamless, integrated and multidisciplinary (MDT) care. Works to achieve change to optimise care close to home and reduce dependency on secondary care, working closely with partner organisations and effecting MDT care settings/scenarios where possible.

An exciting opportunity has arisen for an experienced Registered Podatrist to join the Multi-Disciplinary Diabetes Foot Team (MDFT) The focus of this role will be to work closely with our multidisciplinary team (MDT) to support a patient's journey across the diabetes service footerivce across ELHT and BwD.

The post holder will jointly manage a caseload of patient's working alongside a highly skilled team of Diabetes Consultants, Vascular Consultants, Orthopaedic consultants, Diabetes Specialist Podatrist, Diabetes Specialist Nurses, Vascualr Nurses, Othortics and Specialist Dietitians. The MDFT provides complex care to patients with diabetes foot issues and supports the education of health care professionals across ELHT and BwD .

This is a 7 day service.

Contribute to development of and implement systems under direction of IDSM/ESP that support high performance and continuous quality improvement throughout clinical sub-specialist area. Participate in recruitment and selection of staff in sub-specialist field and other when required. Participate in local induction programs for new starters and returnees to work as required. Regularly review Service performance with staff within own MDFT and recommend effective action to reward excellence and reduce adverse variations.
