Administrator Stroke Early Supported discharge service

Job summary
To work as a member of the Buckinghamshire Neuro Service ESD Team who provide Early Supported Discharge (ESD). The ESD service enables the accelerated discharge of stroke patients to their home environment, providing specialist rehabilitation in the community comparable to that of an in-patient stroke unit. The team works closely with other key organisations to both ensure a seamless transfer of care from the hospital to home and to provide on-going rehabilitation in a variety of community settings.
To provide comprehensive and confidential administrative and secretarial support to a multidisciplinary clinical team in the specialist area of stroke care. The post holder will liaise with other administrators and clinicians to provide secretarial and administrative support through the whole patient pathway. This will include timely management of correspondence, dealing with telephone calls and collecting data for audit and service improvement including SSNAP.
In addition to supporting clinical processes, you will work with other clinicians and managers to facilitate team management and service review and improvement.
Main duties of the job
To provide effective and efficient administrative support for the ESD Team.
Good communication and organisational skills are essential, previous experience of using electronic patient record systems is desirable.
