Due to an expansion of the community Palliative Care Nurse Specialist team we have opportunities for exceptional individuals to join our friendly and supportive community service.
This is an exciting time to join the team as we continue to develop collaborative working with other specialist palliative care professionals, Primary Care Teams and other sectors including, hospital providers, private sector, voluntary services, social services and any other care service providers.
We are looking for experienced, enthusiastic and highly motivated individuals to take up the posts of Palliative Care CNS This post is suited to a nurse with significant experience of caring for patients with life-limiting illness who possesses advanced clinical, teaching and communication skills.
The posts will be based at Florence Nightingale Hospice or Marlow Hospital and a car driver/owner is essential
Main duties of the job
- To provide specialist palliative care, advice and resources to patients with life limiting illness and their carers, in the most appropriate setting.
- To undertake the comprehensive assessment and management of patients including those with complex presentations.
- Collaborate with other providers of palliative care to work towards providing equity of care across Buckinghamshire
- To work as an effective team member within the specialist palliative care service, collaborating with other specialist palliative care professionals, Primary Care Teams and other sectors including, hospital providers, private sector, voluntary services, social services and any other care service providers.
- To provide education, advice and support to professionals, patients and all carers working with patients.
