Job summary
The Frailty Hospital at Home Virtual Ward is our new innovative pathway which focuses on providing intensive, hospital- level care for acute conditions that would normally require an acute hospital bed, in a patients place of residence for a short episode throughmultidisciplinary healthcare teams.
The Division of Integrated Elderly and Community Care are developing the delivery of this new and enhanced model of care which adheres to the spirit of Hospital at Home. This patient pathway with dedicated investment, training and leadership opportunities will guide and inform future pathways for patients to keep people out of hospital and in their homes by referring into a single point of access.
Main duties of the job
The Lead Clinical Response Practitioner has the day to day responsibility for the effective and efficient clinical and operational management of the Frailty Hospital at Home team who provide support to patients in their own home and care homes following a sudden deterioration in their condition.
The expectation is not just to lead a team but to also carry out the initial clinical assessment of the unwell patient with the support of the GP or a Geriatrician. Your clinical expertise are required to plan and deliver care to the acutely unwell patient. Also lead in a clinical supervision strategy that supports the up-skilling and re-skilling the future workforce.
Because the remit of the role is a blend of community and acute medicine, we welcome applications from Community and Acute Nurses, ACPs, Paramedics and Physiotherapists. We believe that every profession has the ability to demonstrate clinical leadership with the correct training, mentoring and coaching opportunities that we will be providing.
