Job summary

There is something special about caring for people in their own home at the end of their life, this is what as a member of the Florence Nightingale Hospice at Home team you would be doing. We are expanding our team to be able to care for more people in the Aylesbury and Buckingham area. We are looking for carers with experience of caring for patients who are reaching the end of their life. We are looking for compassionate carers who can offer a high quality of personal care to the patient but also able to support the family and who can work with other professionals to ensure that the care is seamless.

There a multiple vacancies to allow us to expand to a 24/7 service .

Main duties of the job

Main duties of the job

  • To work in conjunction with Adult Community Health and Specialist Palliative Care nurse teams to provide hands on care in the community setting to patients who are nearing the end of their lives.
  • To provide personal and social care for patients with life-limiting illnesses
  • To work with community care to support people to die in the place of their choosing, avoiding unnecessary admission of patients at the end of life.
  • To provide psychological support for patients and their families/carers.
  • To work as an effective team member collaborating with the other health and social care teams.
  • To assist in the timely discharge of patients from the hospital or hospice, this may be because the patient suddenly wants to die at home or as part of a planned Fast Track discharge.
  • Participate in weekend rotas /7 day working rotas as required

Essential
