Job summary
A new, innovative and exciting opportunity has arisen for a Therapist (Occupational Therapist or Physiotherapist) on a 24 month fixed term contract to work as a Clinical Care Co-ordinator alongside the Team Lead for the Long Covid rehabilitation team. The team consists of Occupational Therapists, Physiotherapists, Psychologists, Mental health specialists and Doctors!
The co-ordinator will be responsible for triaging people with a Long Covid diagnosis with a Personalised Care approach to attend the assessment clinic, appropriate rehabilitation and work clinically at times to support patients back to activities that matter most to them!
You will support the service to triage clients and clients to design their own plan around their health and wellbeing and what they need their goals to be. The client is in partnership with the Therapists and Interdisciplinary team devising a structure of graded steps and pacing to gradually achieve What Matters to them.
This requires experience in Chronic Pain and Fatigue Management, Personalised care and Cognitive Behavioural therapy and an understanding ofmedical, respiratory, neurological, cognitive, mental health, and pain and fatigue strategies.
This is a very friendly, supportive and dynamic department with integrated individual supervision, appraisal, CPD and in-service training.
Main duties of the job
- To participate as an Advanced Clinical Care Co-ordinator for the Long Covid service and contribute to its function.
- To undertake specialised assessments and treatments of patients with highly complex and diverse presentations and to be the point of contact for patients and their families/carers where information is required about the intervention.
- To undertake advanced, comprehensive, and standardised assessments, where applicable, within the specialised clinical area.
- To request completion of assessments by a relevant MDT team member or support team members in the completion of assessments.
- To determine the most appropriate setting for assessment and or treatment. e.g. online or elsewhere.
- To be responsible for organising and planning workload and sendingreferrals to other members of the team highlighting and solving issues if appropriate to meet service and patient priorities, readjusting plans as situations develop.
- To demonstrate advanced clinical reasoning skills, experience andknowledge of evidence based practice to determine treatment plan using a wide range of treatment skills and options.
