Project Manager - Making Every Contact Count

Job summary

An exciting opportunity has arisen for a Project Manager to support with Making Every Contact Count (MECC) across ESNEFT. This post is Fixed Term/Secondment until 31st March 2027.

You will continue delivery on the roll out of Making Every Contact Count (MECC) trust wide, as outlined in the Health Inequalities Strategy. Working collaboratively with partners across the Integrated Care System (ICS) and putting patients at the heart our approach.

  • To further develop the MECC approach across the Trust and community settings, in collaboration with our ICB and community partners.
  • To support, facilitate, monitor and improve progress of the MECC project outputs and outcomes

  • To support implementation of the projects contained within the Health Inequalities Programme by aligning with MECC and Quality improvement (QI) approaches.

ESNEFT Employees will be offered on a secondment basis.

Main duties of the job

  • Working with the Clinical Leads, partner organisations and other key stakeholders to drive forward the delivery of MECC
  • Network and liaise with health professionals and others within the Trust, providing specialist knowledge and expertise around MECC including advising the Trust's Executive Sponsor, Clinical Lead and clinical champions
  • Create all project documentation as required in line with recognised project management principles, ensuring all aspects of the Project management best practices are met
  • Support and attend the Trust's Inequalities Steering Group by providing progress updates on the MECC project. Attend Integrated Care System (ICS) meetings as required.
  • Respond to and ensure the continuous development the MECC project is in line with national guidelines/policy and patient/staff feedback.
  • Analyse public health and population management data to inform project plan development
  • Contribute to the development of care pathways, clinical guidelines, protocols and procedures. Including, as needed, adaptations to patient documentation, template care plans and the electronic patient record templates

  • Educated to degree level or with the equivalent professional experience
  • Evidence of continuing professional development

  • Clinical qualification
