Job summary

An opportunity has arisen for a fulltime Band 7 Lead Nurse within the Stowmarket Community Integrated Team.

Based at Stowlodge in Stowmarket you would be expected to manage and Lead a team of Community Nurses and support staff to meet the nursing needs of the local community.

Main duties of the job

Responsibility for the provision of nursing services within the Integrated team to the local community. Including the development and implementation of new initiatives within nursing services here in Stowmarket.Responsibility to support, maintain and develop best practice among the team to ensure quality care provision to patients and their carers and families.Responsibility to manage and oversee the complex cases liaising with specialist services and multidisciplinary team to ensure a coordinated care approach for patients while supporting staff to feel able and confident to care for such complexity of patient need.Ability to act as a role model motivating and empowering staff within the Nursing and wider INT team.Expectation to support further development of the Integrated Team welcoming new services and staff joining the team. Working professionally with different disciplines, professionals and organisations to achieve the best patient outcomes.

Essential

  • Experience leading a community team
  • Proven years' experience practising as a qualified District Nurse or equivalent
  • Management of complex patients, sharing within the Integrated Neighbourhood Team to foster and grow knowledge and skills
  • Advanced Clinical assessment skills
  • Awareness and understanding of local and national priorities in relation to patients with long-term conditions
  • Ability to manage people
  • Provide and role model high standards of care
  • Work within the Alliance to identify and drive service developments and improvements
  • Excellant communication skills

Desirable
