Job summary

An exciting opportunity has arisen for a Discharge Coordinator to join the Transfer of Care Hub team at Ipswich Hospital.

As a discharge coordinator you will work in close conjunction with the multidisciplinary team and trained staff to progress the patients' pathwaythrough the hospital to expedite their discharge in a safe and timely manner.

This role is full time and has potential of covering 7 days a week but this can be discussed further at interview stage if successful.

Main duties of the job

  • To support clinical areas to progress the patient's pathway through the organisation reducing any delays
  • To meet the needs of the Trust with regard to Induction, mandatory and statutory training
  • To meet the direct and indirect care needs of patients under the delegation and supervision of a Registered Nurse
  • To ensure the delivery of high standards of patient care at all times, in line with Trust policies and guidelines
  • To work as an effective member of the nursing and wider multi-professional healthcare team
  • To work within the boundaries of the role and refer all other matters outside of their scope of practice to the Registered Nurse

  • Proven experience as elective flow assistant in a surgical area.
