Job summary

An exciting opportunity has arisen for a Senior Physiotherapist within the Woodbridge Integrated Neighbourhood Team (INT) on a permanent basis at 22.5 hours per week.

Due to this post being a community position it is essential for the successful candidate to be able to drive from day one of employment.

The Woodbridge Integrated Neighbourhood team is based in Melton and provides multidisciplinary care to the surrounding locality. The team consists of Occupational Therapists, Physiotherapists, District & Community Nurses, Assistant Practitioners, Generic Workers, and Social Workers. The team is led by an INT Manager, supported by a Care Coordinator and Administrator.

The team is busy, dynamic, and proud of their holistic care approach. The team continues to change to support developments in NHS which includes close working relationships with the local statutory, non-statutory and voluntary services. You will be actively supported by regular supervision and individual annual appraisal. You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to the post. You will be required to work shifts covering the operational hours of 08:00 to 18:00 over 7 days.

Main duties of the job

To effectively triage and schedule all therapy referrals to the service in accordance with Trust's policies and procedures and advise junior colleagues on more complex issues.

To manage a caseload of patients with a range of multiple and complex health needs.

To utilise a high level of clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.

To formulate an individual management/treatment plan for each patient using wide range of advanced assessment, clinical reasoning and treatment skills, including health coaching and promotion of self-management skills.

  • Clinical educators course within the first year
