Job summary

We are recruiting an enthusiastic, forward thinking team player to join our dynamic Community Rehabilitation service. We are looking for an Assistant Practitioner who is motivated to achieve their full potential.

It is preferable that you have experience of working in a community or therapy setting although we would welcome applicants with all levels of experience.

It is expected that you will have excellent communication skills, enjoy working with a variety of service users of different age groups, and have good self management skills. You need to have a flexible approach and have the ability to work autonomously as well as part of a team.

The Community Rehabilitation Team is busy and continues to give a high level of service to the patients within North East Essex. We have a high degree of diversity and specialism within our team. We offer a well-established supervision and appraisal structure and we have a firm commitment to CPD, evidence based practice, undergraduate and postgraduate education.

It is essential that you are a car driver as you will be expected to travel across the North East Essex area. This post will be based in the Tendring area, including Kennedy House in Clacton and Harwich Hospital.

The core hours of work are Monday-Friday 9-5, with occasional weekend working. Whilst this is a full time role, part-time hours may also be considered.

Main duties of the job

You will be required to undertake various duties/tasks to support the delivery of person centred care in the community. Enabling individuals to remain at home safely, avoid unnecessary admissions to hospital, and improve their health and independence.

You will work in collaboration with registered therapists as well as support staff and administrators.

Your core duty will be to support exercise programmes and the progression of mobility and functional activities in patient's homes or care settings. Other duties will include assisting in exercise classes, issuing and delivering equipment, and providing assistance with complex, intensive rehabilitation tasks.
