East Suffolk Community Team are recruiting for a part-time peripatetic community nurse. We are seeking a caring and motivated nurse to join our professional and dynamic teams.
It is expected that you will work across the multiple Ipswich nursing teams. You will be committed to flexible team working, delivering high quality patient care.
You will receive daily allocation of their visits, depending on where the service demand is greatest.
This is an opportunity to work within an Integrated Health and Social Care Team consisting of Nurses, Physiotherapists, Occupational Therapists , Generic Workers, Social Workers and IWP's undertaking holistic assessments and treatments for patients within their own homes. You will be using a wide range of nursing skills and will need to be confident in their assessment skills and in their ability to provide safe, effective treatments whilst working in a community setting.
Experience of palliative care, wound management, venepuncture and catheter management will be an advantage. You should be flexible, motivated and able to work within a changing environment.
You will undertake a comprehensive induction to introduce you to the role. We offer a variety of training in order to up skill and maintain clinical skills. You will be offered a tailored induction depending on their experience.
Main duties of the job
- To work flexibly across all of the Ipswich nursing teams.
- Triage, assess, plan, implement and evaluate all aspects of patient care, and develop care plans which may need to be delivered from a range of options
- Ensure informed consent is obtained and documented prior to initiating interventions
- Following a holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required
- Assess patients' holistic needs, communicating complex and sensitive information to patients and carers
- Use tactful and persuasive skills to gain the patient's co-operation in their care management plan
- Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated
- Maintain skills with regards to latest technology
- Participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework
- Ensure own workload is planned and prioritised according to patient needs
- Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis
- Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills
- Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions & adhere to infection control policies
