Job summary

An exciting opportunity has arisen for a Physiotherapist within the Woodbridge Integrated Neighbourhood Team (INT) on a permanent basis at 22.5hours per week.

We are looking for a Physiotherapist who is passionate, committed, innovative, enthusiastic, and highly motivated to delivering high quality care within a community setting. Our service runs 8 am-6 pm, 7 days a week. Staff are required to work approximately 1 in 10 weekends, with flexible working available within the hours of the service.

The Woodbridge INT is based in Melton and provides multidisciplinary care to the surrounding locality. The team is committed to holistic and patient centred care in the right place, at the right time. The team consists of Occupational Therapists, Physiotherapists, District & Community Nurses, Assistant Practitioners, Generic Workers, and Social Workers. The team is led by an INT Manager, supported by a Care Coordinator and Administrator.

Although we expect our staff to work autonomously, we're a very supportive team and work closely together to achieve the best outcomes for our patients. Community experience would be an advantage, but the role includes a comprehensive trust induction followed by a local team induction. We offer a variety of training to consolidate and maintain your clinical skills.

Please note this role does require proven NHS experience.

Main duties of the job

You will be actively supported through in-service training and professional forums, mandatory training, 1:1 personal development meetings, as well as clinical skills training via the practice development facilitators.

To provide high standard patient care in patients homes

To ensure a more seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment

To provide supervision and support to more junior staff

To triage referrals to the community team

Due to this post being a community position it is essential you are able to drive.

Qualifications

Essential

  • Diploma or 1st Level degree in Occupational Therapy (or equivalent)
  • Current HCPC registration
  • Clinical educators course within the first year

Knowledge & Skills

Essential

  • Knowledge of additional specialist areas through in-house training & short courses
  • In-depth knowledge of working within the NHS
  • Advanced observational & reporting skills
  • Able to demonstrate good IT and standard keyboard skills
  • Must hold a full valid driving licence and have access to a vehicle or viable alternative
  • Able to support other staff to review and reflect on their practice and develop a PDP

Desirable

  • Able to demonstrate the ability to work within a community health team
  • Able to demonstrate the management of complex casework

Experience

  • Documented evidence of continuing professional development.
  • Post registration experience in a relevant clinical setting.
  • Proven NHS experience
