Job summary

Aldeburgh Community Hospital has an exciting opportunity for a static band 6 Physiotherapist (up to 37.5 hours). Aldeburgh Community Hospital is one of three East Suffolk Community Hospitals alongside Bluebird Lodge and Felixstowe Community Hospitals working in close partnership with the Ipswich Hospital and as part of the wider ESNEFT NHS Trust. The role is be based at Aldeburgh Community Hospital with the opportunity to experience working at all three community hospital sites.

We are looking for a talented Physiotherapist with commitment and enthusiasm, who has a passion to work as part of a team to progress patients rehabilitation goals and use a range of therapy interventions to facilitate timely and appropriate discharges.

You will work as part of an existing therapy team to promote the discharge to assess pathways for in-patients working alongside the multidisciplinary team of Modern matron, ward nurses, social care workers and Ipswich Hospital Discharge Hub. You will participate in weekend working to facilitate a seven-day service of 1 in 4 weekends and work some of the bank holidays.

Main duties of the job

  • To be responsible alongside the multi-disciplinary team for daily caseload management of therapy interventions ensuring all patients receive timely, appropriate, effective treatment and discharge planning.
  • To work with patients and carers to identify Physiotherapy goals, using a wide range of Physiotherapy assessment tools to assess their skills deficits and rehabilitation needs
  • In collaboration with the patient, planning and implementing single or a programme of interventions using treatment techniques to meet the patient's needs. These will be achieved using professional and theoretical knowledge, broad clinical knowledge, and clinical reasoning.
  • To apply understanding of effects and impact of illness and disability on patient's abilities and provide advice and education regarding lifestyle changes / adaptations to their physical and social environment
  • To participate in own and others continuous professional development, appraisal processes and other developmental activities according to professional registration and trust policies.
  • To supervise and assist in the development of junior clinicians, non-qualified staff, and students on placement.
  • To undertake audit of both clinical practice and processes to increase efficiency and effectiveness.

  • Post registration experience to include therapy rotations at B5 and B6 level across acute / community / social services care to include a spectrum of health and social care conditions
  • Experience of managing frailty
  • Experience of facilitating discharges for people with complex needs and involving multi-pathology.
  • Experience of complex caseload management
  • Documented evidence of continuing professional development involving specialist additional knowledge and experience
  • Experience of supervision and professional/clinical development of staff
  • Experience of demonstrating moving/handling skills including equipment
