An exciting opportunity has arisen within the Ipswich & East Suffolk Integrated Neighbourhood Teams. We are looking for experienced band 6 Occupational Therapists / Physiotherapists who are passionate, committed, innovative, enthusiastic and highly motivate to delivering high quality care within patient's homes.
There are a range of positions available and opportunities to work in the following teams (Ipswich East, Ipswich West, Woodbridge, Felixstowe, South Rural & Stowmarket) the applicant will be asked at interview where their preference location will be.
This is an opportunity to work within a knowledgeable experienced team and wider MDT. You should be flexible, motivated and willing to work within a changing environment. We will provide support through in-service training and professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring community teams in Health and Social Care.

Main duties of the job
- To provide high standard patient care in hospital settings and patients homes
- To ensure a more seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment
- To provide supervision and support to more junior staff
- To triage referrals to the community team
