Job summary

An exciting opportunity has risen for an enthusiastic and motivated individual to join our Integrated Urgent Crisis Response Service (IUCRS) as Advanced Clinical Practitioner Allied Health Practitioner (AHP), providing a holistic approach to the care of individuals.

37.5 hours per week

The IUCRS team is a multi-disciplinary team based in Kennedy House Clacton-on-Sea, with other bases in Colchester.

Our team delivers high quality care,7 day a week within the North East Essex community. We will offer individually tailored induction, training, and support for you.

The role involves travelling throughout North East Essex delivering, assessing, and treating patients in their own homes, within 2 hours, as a part of IUCRS Urgent Community Response Team (UCR). You will work closely with the East of England Ambulance service (EEAST) to prevent unnecessary conveyances to hospital. The core hours of the service are 08.00-22.00 and you will also be expected to work weekends and bank holidays.

It is essential that you have a valid driving licence.

Main duties of the job

You will provide professional leadership, clinical expertise and manage urgent presentation of patients in a crisis and admission avoidance situation within North East Essex Community.

As a lead clinician of this service you will be expected to work in partnership with primary and secondary care colleagues and other agencies to secure a smooth patient journey across the whole patient pathway. Self-care will be encouraged, and patients and carers will be actively involved in the development of their care plan and the management of their condition.

You will also act as a role model while promoting clinical effectiveness and delivering advanced practice.

IUCRS is an evolving and constantly developing team incorporating the following areas: Urgent Community Response (UCR), the cleric ambulance system (working in conjunction with EEAST), virtual frailty ward, in reach into the Integrated Rapid Assessment service (IRAS) based in the Emergency Department Colchester General Hospital, Unplanned Community Nursing and OPAT IV antibiotic service. You will be expected to perform comprehensive assessment of patients with urgent presentations and develop working diagnosis and treatment plans whilst working without direct supervision, with virtual input from the medical team.

  • oEvidence of continued professional development following MSc completion.
