Job summary

We have a really exciting secondment opportunity for somebody who has the passion to support the transformation of neighbourhood working within the Tendring area.

The post-holder will work in collaboration with the Colchester Neighbourhood Manager, the Head of Transformation of the Neighbourhood Programme and the NEECS division General manager for Neighbourhood Working to embed this model of care into the three Tendring localities.

The Neighbourhood Manager's role is to mobilise, manage and lead the Live Well Neighbourhood Team. The successful applicant will work in collaboration with system partners to develop a neighbourhood plan for the population of Tendring South.

Live Well Neighbourhood Teams (LNT's) will build on a vibrant social, voluntary and community sector to support individuals to look after their health and wellbeing, connect with others, manage their long-term conditions and stay independent for longer. It will be delivered through a designated multi- disciplinary team, bringing together people from a wide range of organisations including health, social care, public and voluntary sector organisations.

Main duties of the job

The Neighbourhood Manager is responsible for mobilising and establishing an integrated Live Well Neighbourhood Team and leading a team of system partners to work together to;-

Address the wider determinants of health that are of greatest concernto the community (e.g.poor housing)which will help to reduce future ill health in the local population.

Help people living with long term conditions to address risk factors,effectively manage their self-care and reduce their risk of worsening disease.

Identify the most vulnerable people in society and provide support at an earlystage, to prevent future emergency call outs and hospital admissions.
