The Heart Failure Service at Milton Keynes University Hospital is a dynamic service that cares for patients across the in-patient / outpatient pathway and in particular is dedicated to the management of patients presenting acutely to the Trust.

We are currently looking for a part time, Band 7 cardiac advanced nurse practitioner or clinical nurse specialist to join our established team. Ideally the hours will be split over 2 days Monday and Wednesday mornings, but there is the potential for candidates to work Monday one week and Wednesday the following week.

Your duties would include: assessing patients acutely within the trust, acting as an autonomous practitioner in outpatient clinics, and managing the diagnostic and treatment pathways for patients diagnosed with acute or chronic heart failure.

This exciting role will provide the successful candidate with a dynamic skill set and the opportunity to work within a multi-professional team to improve the care of the patients we serve. They should have proven experience of cardiology and experience in caring for Cardiology patients.Excellent communication and interpersonal skills are essential.

The post holder will act as a highly visible resource for expertise in Cardiology nursing for all service users, demonstrating clinical competence and confidence using highly developed specialist knowledge and skills in a variety of clinical settings.

To work independently and without direct supervision in a variety of settings Trust wide, including inpatient areas and outpatient clinics, demonstrating the ability to prioritise and manage workload.

Professional

To be a point of contact and clinical expert for patients, relatives and clinician colleagues in conjunction and with the full support of the Consultant Cardiologists. The post holder will be expected to ensure that the patient receives the highest possible standard of evidence-based care in line with current NICE and ESC guidance on:-

o Chronic heart failure in adults: Management (2010)

o Myocardial Infarction with ST segment elevation (2013)

o Acute Heart failure: Diagnosis and management (2014)

o European Society of Cardiology Guidelines for the diagnosis and treatment of acute and chronic heart failure (2016)

o Cardiovascular Risk assessment and management.

Clinical Duties

Perform comprehensive patient assessments based on history taking and physical examination to arrive at an accurate nursing/medical diagnosis. Independently act accordingly suggesting solutions, making recommendations and creating management plans.

As appropriate, order diagnostics.

Prescribing

To formulate and implement treatment plans utilising nurse prescribing skills, based on current evidence and expert clinical reasoning with a comprehensive understanding of available treatments to support an improvement in quality of life and or independence.

Staff management / leadership / training

Develop patient education programmes and promote health education particularly in line with NICE guidance.

Act as a role model demonstrating professional clinical leadership.

Managerial

To maintain accurate and up-to-date records and reports of patients being treated in line with departmental standards and the appropriate professional body. Guidelines and standards for record keeping as per the current NMC Code of Conduct.

  • NMC Registered General Nurse, educated to at least first degree level
  • Validated Post Graduate Qualification Level 6 or equivalent in Cardiac care - 124 or equivalent
