We are seeking an experienced and motivated Diabetes Specialist Nurse with expertise in insulin pump therapy and technological diabetes management to join our dynamic multidisciplinary team.

This is an exciting opportunity to work in a progressive diabetes service committed to excellence in care and innovation in treatment. You will play a key role in supporting patients with Type 1 diabetes using insulin pumps and continuous glucose monitoring (CGM) devices, including hybrid closed-loop systems.

Main duties of the job

To provide expert care and advice to all patients with different types of Diabetes who attend the hospital either as an inpatient or outpatient.

To reduce the impact of symptoms and improve the quality of care for patients with diabetes, including optimising treatment and providing education and support for patients and their families and carers.

To take the lead role in the development and delivery of expert education and training in the area of diabetes for other health professionals and stakeholders. To act independently and when necessary in collaboration with the multidisciplinary team to ensure patients receive optimal care.

To be recognised as the clinical expert for diabetes representing the acute trust to

external agencies. To be able to disseminate appropriate information to others across

the acute trust.

To provide a seven-day service to persons with diabetes

To provide a seamless service for those who are vulnerable i.e., during pregnancy and

young adults.

To provide leadership, demonstrating transformational leadership skills in the area of expertise, developing the service in conjunction with the multidisciplinary team across the agencies / disciplines.

To act as an expert practitioner and role model, demonstrating advanced skills and knowledge to professional staff and patients.

To analyse and interpret national recommendations, i.e., NSF/NICE guidance and formulate action plans to achieve compliance from a nursing perspective.

To co-ordinate and implement research projects and disseminate the findings. Developing a research culture within the multidisciplinary team, working towards reducing the practice-theory gap.

To promote expert practice, facilitating the development of others by acting as a role model, mentor, and supervisor in practice, also providing advice to the multi-professional team across the range of agencies and disciplines.
