Tissue Viability Specialist Nurse

Department: Tissue viability Service

Band 7

Hours: 37.5 hrs per week, all MKUH roles will be considered for flexible working

An exciting secondment opportunity has arisen for a motivated and enthusiastic registered nurse to join the Tissue Viability Service for maternity cover. This will be a developmental opportunity for senior nurses with a keen interest and background in tissue viability service. The successful candidate will work alongside the team to embed and shape outstanding quality care.

This role will require you to assist and work independently within the Tissue Viability Team. You will actively contribute to the prevention of avoidable pressure ulcers, contribute to the management of complex wounds and provide advice on continence care. You will deliver training on all aspects of Tissue Viability to patients, carers and all internal staff groups, as well as other quality training as required.

We are looking for someone who can make a difference to patients and staff, modelling outstanding care every time and able to promote a positive team working ethic. You will have excellent interpersonal and communication skills, be highly motivated and flexible in your approach to care delivery and be committed to quality improvement

Main duties of the job

To provide clinical leadership and training for clinical staff in all aspect of tissue viability care

To assess the current treatments, rationalise resources, improve clinical outcomes and monitor cost effectiveness in care provision within the specialised area of Tissue Viability.

Develop specialist policies, clinical guidelines and care procedures relating to Tissue Viability to support the development of the service provision.

To improve quality, health outcomes and service delivery by working collaboratively with internal multidisciplinary teams and external agencies across professional and cross organisational boundaries.

To provide specialist knowledge/advice to medical and nursing staff on individual clinical care to patients with compromised skin integrity or who are at risk of it.

To provide advice on continence assessment, management, and appropriate use of incontinence products

To advise and facilitate the planning and delivery of specialised care packages for patients with compromised skin integrity or who are at risk of it.

To participate in the provision of specialised training and education on tissue viability to all levels of staff and disciplines.

To provide leadership in the development of Tissue Viability Services in line with the current priorities of the Trust.

To review the current use of pressure-relieving support surfaces and wound care modalities in order to ensure their use is cost effective.

Main duties and responsibilities:

Work as a Senior Nurse within the NMC Code of Conduct and Scope of Professional Practice.

  • Participate in the planning and delivery of Tissue Viability services within MKUH working collaboratively with members of all care groups and Infection Prevention and Control to improve the Health and Safety issues in Tissue Viability, in particular work towards the reduction of New pressure ulcers/wound infection rates.
  • Provide support, guidance, and advice to members of the multidisciplinary team, imparting expert knowledge and information regarding wound therapy specific to each individual patients needs and requirements.

Education and Training:

  • Identify training and education requirements and to develop training and education programmes to promote best practice in Tissue Viability within the Trust.
  • To review and update training and education packages at least annually in line with National agendas.
  • Provide support and supervision to clinical staff on their professional and personal development relating to Tissue Viability.

Patient Care:

  • Provide patients and relatives/carers with information on pressure relieving and wound care products and provisions. This should be in a number of different formats to ensure that patients and carers understand the information they are being provided with.
  • Assess patients suitability and appropriate usage of pressure-relieving equipment.
  • Participate in Multi-Agency Discharge Planning meetings to advise on wound care and pressure relieving products being supplied to go into the patients own home upon discharge.

Clinical Governance:

  • To collate data regarding pressure ulcer incidence from all clinical areas
  • To provide statistical reports for various forums (Corporate, Divisional and Trust Board) using various computer software packages .
  • Audit the use of pressure relieving equipment to ensure cost effectiveness.

Communication:

  • Demonstrates and establishes a relationship of Trust with patients carers and families.
  • Demonstrates expertise when dealing with complex patients with comorbidities and their families who require new skills to live with a chronic condition.
  • Ensures the rights of the patient are acknowledged and respected. Can act as an advocate on the patients behalf.

Resource Management:

  • Responsible for appropriate wound care and pressure relieving products to be purchased in liaison with the Procurement team.
  • Responsible for providing product feedback on wound care and pressure-relieving products.
  • Responsible for the tracking of certain agreed wound therapy equipment.
