Job summary

Applicants are invited to apply for a LAS ST1-2 post in Medicine at Milton Keynes University Hospital NHS Foundation Trust. It is envisaged this post would be suitable for a doctor who has completed foundation training or has 1-2 years of postgraduate experience as a doctor and would like the experience of a rotation in the general medical specialties in a busy acute medical hospital. This is a fixed term post from 2nd October 2023 until 30th June 2024.

This is an exceptional opportunity for a dynamic doctor who wishes to develop their skills as a physician and perhaps would like to be supported to study towards MRCP examinations. For most weeks work will be 08:00 to 17:00 hours, Monday to Friday. When doing shift work (2 weeks in 6) the rota has been redesigned to improve the working life of junior doctors and does not allow long stretches of work and allows ample time to recover between shifts.

Milton Keynes University Hospital is the major teaching hospital for Buckingham University medical school. Recently Buckingham University built a state-of-the-art academic centre on the Milton Keynes University Hospital site.

Main duties of the job

The job will involve:

  • A 6 week repeating work cycle consisting of:
    • 4 weeks of ward work (08-17:00) on a general medical ward without any out of hours work,
    • 2 weeks of shift work consisting of day and night shifts approximately 9 shifts in 14 days.
  • Opportunity to rotate through a variety of medical specialities including Acute medicine, General medicine, elderly care, cardiology, respiratory, gastroenterology, haematology, diabetes and endocrinology.
  • Opportunities to do research, audit or quality improvement projects.
  • Yearly Appraisal with a nominated consultant and access to appraisal software.
  • Opportunity to deliver undergraduate teaching to Oxford and university of Buckingham Students.
  • A Clinical / Educational Supervisor for career development.
  • Access to eportfolio to record competencies and document clinical experience.
  • Weekly in house teaching sessions and dedicated MRCP PACES teaching with RCP examiners.

There will be many opportunities for professional development and you will be allocated a clinical supervisor to help with a personal development portfolio. Applicants will also be supported in applying for National Training Numbers (NTN's).

A study leave budget of £600 per year and allowance of 10 days will be available. An annual leave allowance of 27 days is given with compensatory rest days for on calls.

Job Profile

The LAS ST1-2 will act as a junior grade doctor under supervision at all times. Supervision may be from a Registrar, senior specialty doctor and/or Consultant. It is envisaged that this post would be suitable for a doctor with 1- 2 years postgraduate experience or after foundation training.

Job Outline

The successful candidate will rotate through a variety of medical wards and specialities gaining exposure and education in the areas of general and specialist medicine. Ward placement would usually be for 4-6 months and could involve rotating through general medicine, elderly care, respiratory, diabetes and endocrinology, haematology, cardiology, gastroenterology and acute medicine. He/she will be expected to participate in most of the day to day activities of the department in cooperation with the rest of the medical staff.

In addition there will be involvement in the out of hours rota paid in additional allowances per 2016 Junior doctor contract. In August 2019 we changed the way the on call is designed. The new out of hours rota is now a full shift rota that allows for 4 weeks of ward based (08:00-17:00) clinical activity followed by 2 weeks of intermittent shift work comprising a mixture of days, weekends and night shifts. The rota was designed to significantly improve the work life balance of junior doctors and we have worked hard to ensure that there are no long stretches of work without a break. Currently this rota averages 44 hours per week when averaged over the year but may be subject to minor changes over the course of employment.

The job is mainly ward based but the candidate will have educational exposure to outpatient clinics and the secretarial support to achieve this.

We recognise that education and career progression is an essential requirement for doctors not in a recognised training programme. Each doctor will have a nominated clinical and educational supervisor, with whom to meet and discuss progress and access to the same eportfiolio as colleagues in training to document their competencies and educational progression. They will also have access to the hospital appraisal system to ensure they meet the GMC requirement son revalidation. They are encouraged to take appropriate study leave and also attend the weekly in house teaching suitable for their stage of training.

In 2015 the University of Buckingham joined with Milton Keynes Hospital to create a medical school. The first undergraduates graduated as doctors in August 2019. A major new academic centre was built on the Milton Keynes University Hospital Campus in 2018. Undergraduate teaching is strongly encouraged and there will be the opportunity for the successful candidate to apply for funding to undergo a formal teaching qualification eg PGcert (Medical education).

The successful candidate will be responsible to work under the supervision of the Consultant Physicians within the department of medicine. The post holder will perform, with supervision, procedures appropriate for their level and experience. They will also be expected to be competent in all general areas of the specialty consistent with training and experience and to cover the work of colleagues as appropriate. They will be expected to actively participate in educational activities including teaching, grand rounds, case presentations and audit.
