Cancer Navigator

An exciting opportunity has arisen at Milton Keynes Hospital NHS Foundation Trust to join the Cancer Navigator team working within cancer services (Upper GI and Head and neck cancer).

We are looking for someone with excellent communication skills, both verbal and written. You should have keen attention to detail and good organisational skills. The successful applicant will have excellent interpersonal skills, enjoy helping people, be a team player and have an approachable attitude.

Main duties of the job

  • To act as the patients navigator tracking their timed pathway against the national guidance
  • Responsible for entering accurate data onto the patient access system / Somerset cancer data base via Consultant pro formas / instructions, following the patient access policy, in accordance with patient choice.
  • Central communication point for patients on cancer pathway , ensuring that patients (and their families) are supported and clear on next appointments
  • Act as a point of contact to listen to patients' queries and concerns and pass then to the appropriate areas and escalate to clinical teams as required
  • Ensure that all patient entries are up to date and accurate, escalate concerns over delays to Cancer Manager
  • Supporting the Specialist Nurses with nurse led clinic bookings and preparation

Job description

Job responsibilities

  • To work in conjunction with the MDT team to ensure patients, GPs and referring hospitals are aware of the progression of treatment (i.e. dates and results of tests, operation etc.).
  • To link in with Cancer trackers in the region and allow a continuous and productive exchange of relevant information.
  • Establish effective and collaborative working relationships with the key staff.
  • To monitor time intervals between points of treatment, pre-empting and identifying delays and act upon these appropriately.
  • To monitor and track PTL waiting lists within the speciality and ensure accuracy and completeness.
  • Ensure all clinical outcomes recorded are checked and outcomes are actioned to avoid administrative delays in the patient pathway.
  • Put in place arrangements to monitor progress against targets to ensure daily compliance with guarantees.
