Our Specialist Palliative Care Team integrates high-quality palliative and end-of-life care across the Trust. As a Clinical Nurse Specialist, you will act as an expert resource, supporting patients with both cancer and non-cancer diagnoses through some of the most challenging moments of their lives.

You will be a visible clinical leader, role model and advocate for excellence in palliative care, working collaboratively to enhance patient experience and care outcomes across all adult areas of the RUH.

Main duties of the job

In this varied and fulfilling specialist role, you will:

  • Work autonomously to assess palliative and end-of-life care patients, including history taking, holistic assessment, planning care and evaluating outcomes.
  • Provide expert advice and support to patients, families and multidisciplinary teams
  • Lead and contribute to education for staff, promoting high standards of palliative and end-of-life care practice.
  • Support quality improvement initiatives, helping drive service development and best practice.
  • Collaborate withSpecialist Palliative Care Team and support the Lead Nurse in service delivery.
  • Work across adult clinical areas where patients with palliative needs are supported.
  • Participate in our 7-day Specialist Palliative Care service, helping ensure consistent support.
  • Perform advanced clinical skills in assessment of palliative care or end of life care patients including history taking, holistic clinical assessment, advising on management plan and evaluation of care.
  • Put quality at the heart of practice by delivering evidence based individualised and person-centred care.
  • Monitor and review the effectiveness of interventions with patientS & modify this to meet the changing needs and established goals of care.
  • Prescribe and review medication for therapeutic effectiveness, in accordance with evidence- based practice and national & practice protocols & legislation within scope of practice.
