Join Our Team as a Discharge Coordinator at the RUH

We're looking for an organised, proactive and compassionate individual to join us as a Discharge Coordinator. In this vital role, you'll help ensure patients have a safe, smooth and well-planned transition from hospital to home or onward care.

As a key member of our ward teams, you will:

  • Work closely with clinical staff to coordinate timely and effective patient discharges
  • Manage and update patient information on Millennium to support accurate planning
  • Support the collection and reporting of business performance data
  • Play an active part in improving patient flow across the hospital, helping us deliver responsive and high-quality care

Main duties of the job

Coordinate timely and safe patient discharges by ensuring effective communication across the system.Update patient information accurately on Millennium to support discharge planning.Work collaboratively with the Multi-Disciplinary Team to prepare clear and accurate referrals.Support a designated cluster of wards, assisting additional wards when needed to maintain optimal patient flow.Ensure smooth and consistent communication of discharge information between RUH wards and community partners.Attend key meetings, handovers and relevant training sessions.Handle patient information sensitively, ensuring all data--especially confidential information--is managed in line with Information Governance standards.Develop a strong understanding of discharge processes (e.g., transport arrangements) and how these link to electronic systems such as Millennium.Provide comprehensive administrative support to ward staff to facilitate efficient discharge processes.

Recognising training needs independently and acting on these,informally or formally as needed.3. Following written and verbal training information on theprocesses involved in the administrative role.4. To update staff in a timely way if/when changes occur.5.
