Job summary
Cheselden is an elderly care ward specializing in the rehab and care of patients with complex discharge issues.
As a Registered Nurse on the elderly care ward, you will play a vital role in delivering high-quality, compassionate, and person centered care to older adults, many of whom present with complex medical, cognitive, and social needs.
You will be responsible for assessing, planning, implementing, and evaluating individualised care plans, working as part of a multidisciplinary team to promote patient well-being, dignity and independence.
We're looking for an individual who has a caring, kind and professional approach, excellent clinical skills and the ability to respond to the changing needs of patients in a busy and sometimes challenging environment.

Main duties of the job
To work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker (with facilitation) for a defined group of patients and take responsibility for:
The assessment of care and health education needs
The development, implementation and evaluation of programmes of care including discharge planning for each patient.
To gain experience and skills in:
Clinical practice
Facilitation and teaching
Management (as appropriate)
To work in accordance with the RUH Nursing Strategy and contribute towards achieving its objectives and maintain effective communication and participate in clinical supervision as appropriate and to teach and act as a facilitator/mentor/preceptor/ role model to less experienced staff.
