Discharge and Flow Co-ordinator

Job summary
This post is responsible for proactively supporting effective patient flow and both 'simple' and 'supported' discharge at ward level.
The post works closely with the Integrated Discharge Service (IDS) within the Care Transfer Hubs (CTH) and Clinical Site Team (CST).
The IDS is responsible for supporting Wards with Complex Discharges across the RUH Bath. The IDS leads the RUH CTH which is where all discharge referrals are triaged, and discharge pathways agreed alongside community partners / MDT.
The CST co-ordinates the management of inpatient bed capacity to match demand. The ethos of the IDS is to identify patients at ward level who require a supported (complex) discharge, working with patients, families and ward staff whilst patients are not medically stable by planning discharge arrangements and discharging patients as soon as possible once medically stable.
The post holder will work within both a designated ward team and with the IDS and CST teams providing support to all members of the team
Main duties of the job
Main ongoing responsibilities will include the proactive progression of timely and safe patient flow and discharge, minimising avoidable delays, ensuring and maintaining accuracy of information, supporting the ward team to progress chase and ensuring that the ethos of the 'releasing time to care' is maintained. You will be committed to joint working with our other providers and community teams to provide a "why not home, why not today" approach to reduce length of stay and facilitate early discharge.
