Dementia Support Worker

Job summary

To work with all members of the multi-disciplinary team, providing individual support, education and information for patients, relatives, carers, volunteers and staff involved in dementia care.

To enable patients, families, carers, staff and volunteers to have improved access to support, information, education and knowledge on services available, and to receive individual support as indicated in patient centred care plans and to the highest standards of patient care.

To support the delivery of a high quality, safe and compassionate health care service all staff are expected to act as a role model to others in all aspects of their work and consistently demonstrate QEH values.

Main duties of the job

To complete the Abbreviated Mental Test Score (AMTS) for all patients aged 75 and over.

Initiate the Dementia trio and support with completion of "This is me."

Participate in an active in reach information and support service across adult areas for patients living with dementia, their families and carers.

Support patients, carers and families in situations of emotional distress relating to dementia care.

Develop and support activity programs under supervision of trained staff.

Carry out activities and therapies as part of individual patient care.

Assist in delivering and designing training and educational programs for patients with dementia, staff, volunteers, families and carers.

Be a role model for dementia support and liaison.

  • Ability to communicate widely in a clear manner, problem solve with good verbal and written English skills
  • Ability to self-manage time and meet agreed deadlines, organising and prioritising own delegated workload
  • Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice

  • Flexible attitude to service delivery
