Discharge Co-Ordinator

Job summary

To independently actively monitor the multidisciplinary management anddischarge plan for all patients on designated ward(s) and take action to expeditethe process and thereby improve the patients' experience. A critical success factorin this role is reducing the length of stay of all patients. For some patients this willmean reducing stay by hours, for others by days and weeks.

A pivotal part of the role will be the as a Trusted Assessor. A trusted Assessor willundertake a holistic assessment of needs that avoids duplication and speeds upresponse times from Care Homes enabling people to be discharged in a safe andtimely way.

To support the delivery of a high quality, safe and compassionate healthcareservice all staff are expected to act as a role model to others in all aspects of theirwork and consistently demonstrate QEHKL values of Kindness, Wellness andFairness.

Main duties of the job

To have an up-to-date knowledge of the multidisciplinary management anddischarge plans of all allocated patients, updating patients, and theircarers/families as appropriate in liaison with ward multi-disciplinary team (MDT).If a clear plan cannot be identified in the medical record, to contact relevantclinicians for clarification. Expectation is to manage more complex dischargecases with the support of the Complex Discharge Sisters.

To independently gather and collate information from the medical notes,patients and multi-disciplinary team colleagues to enable a clear plan fordischarge to be followed.

To undertake and support others in assessments which facilitate effective and timelydischarges from acute hospital and community settings, ensuring people aredischarged to the right setting with complete, accurate and up to dateinformation and associated documentation. To maintain a strong, visiblepresence providing expertise and assessment skills in acute, community and carehome settings.

To provide data, co-ordinate discharges and provide administrative support tothe multi-disciplinary team within the hospital to improve joint workingpractices leading to more effective patient care and timely discharges.
