Job summary
Under the supervision of the Junior Complex Discharge Sisters (JCDS) undertake agreed duties as below, demonstrating the level of competence and professionalism required as a participating team member to deliver high quality service.
Actively monitor discharge plans for all patients on designated wards and take appropriate action to expedite the process, avoid delays and thereby improve the patient's experience.
Highlight those patients with potentially complex discharge needs to the JCDS in a timely manner. Ensure all patients in designated wards have a current management / discharge plan and an identified, documented 'Estimated Date of Discharge'.
Main duties of the job
- Communicate with patients, relatives, carers and members of the multidisciplinary team - by telephone, face to face to ensure that discharge takes place in a timely fashion.
- Document completed activities in the care plan to ensure a holistic approach and continuity of care for patients.
- Contribute to effective communication within the multidisciplinary team and Complex Discharge Team to ensure the delivery an effective service.
- Participate in meetings and share information with other members of the Discharge
- Planning team to assist in maintaining an informed workforce.
- Ensure communication is carried out in a manner and at a level and pace appropriate to the recipient using interpreters/aids as necessary to achieve desired outcome.
