Job summary
The Palliative Discharge Coordinator role is responsible for assessing and coordinating the discharges of palliative care patients within QEH, this includes the completion of the Fast Track pathway tool for Continuing Health Care in partnership with Norfolk, Lincolnshire and Cambridgeshire NHS Continuing Health Services.
The Palliative Discharge Coordinator role will be to provide support to patients and their families that require support through the palliative discharge planning process and fast-track pathway. Working actively to support patient choices with preferred places of care and preferred places of death as part of the discharge planning process.
To organise, plan and support safe, compassionate end of life care, putting the patient and their representatives at the centre of all decisions, whilst actively working to ensure that any delays in discharging to the patients preferred place of care / death are removed.
The Palliative Discharge Coordinator role will be based within the hospital palliative care team but will work collaboratively with other teams within the organisation to ensure timely and safe discharges for those individuals entering the last stages of life.
Main duties of the job
- Provide support and information to patients and families regarding choices at the end of life.
- Provide education, training and support within the acute hospital to promote excellent end of life care.
- Ensure that data is collected to demonstrate activity and performance as required.
- To promote end of life processes within the acute hospital and being the point of contact for staff.
- To work in partnership with the Complex Discharge Planning Team.
- Develop collaborative relationships with other care providers across health and social care to ensure communication links are in place for a continuous and seamless discharge process.
