The Home Ventilation Team provides specialist care and support to children and young people who require long-term ventilation. The team works predominantly in the community but also reviews acutely unwell patients on the wards and supports consultant colleagues in clinic, delivering a service across the region.

In this role, you will provide specialist nursing care to a varied group of patients across multiple settings, planning and evaluating care using appropriate clinical pathways.

You will also contribute to the development and implementation of policies, guidelines and standard operating procedures to support high-quality, consistent care.

About You

  • You will have experience delivering ventilation care to children and young people, including both invasive and non-invasive ventilation
  • You will be confident providing education and training in a variety of settings, including schools, and able to adapt your approach to different audiences.

Main duties of the job

Reporting into the Clinical Nurse Specialist Manager some of your responsibilities will include:

  • You will be able to write individualised care plans that reflect each child's wider health needs and how these affect their ventilation requirements.
  • You will deliver developmentally appropriate, specialist nursing care to a diverse group of patients across a range of settings, including inpatient, outpatient, day care, community and educational environments.
  • You will plan and evaluate complex care using appropriate pathways, working collaboratively with the multidisciplinary team. This will include running nurse-led clinics alongside consultants, providing telephone advice and carrying out home visits as part of the service.
