We're seeking enthusiastic Home Care Support Workers to support children with complex biopsychosocial needs related to excessive weight. You'll help families make positive lifestyle changes while working as part of a multidisciplinary team--including specialist nurses, consultants, dietitians and psychologists--providing support in both clinic and community settings.
About You - You're confident working independently and as part of a multidisciplinary team alongside consultants, specialist nurses, psychologists and dietitians.
- You bring excellent communication skills, GCSEs in English and Maths (A-C), and an NVQ Level 3 in Health and Social Care or equivalent.
- A full UK driving licence is required.
Main duties of the job
Reporting into the Nurse Specialist Weight Management, some of your responsibilities will include:

Support children, young people and families within the Specialist Paediatric CEW Service across Sheffield, Rotherham, Doncaster and Barnsley.

Provide healthy eating guidance, health promotion and lifestyle advice.

Deliver support in clinics, during home visits and within community settings.

Work autonomously, managing your own workload while contributing to a multidisciplinary team.

Build effective relationships with families to encourage positive lifestyle changes.
