An exciting opportunity has arisen for dynamic and motivated person to join our team on PACU as Senior Clinical Support Worker.
The successful post holder will work alongside or independently with qualified Nurses or Practitioners, including medical staff or the multi-disciplinary team in the delivery of individualised care to children, young people and their families.
The registered Nurse holds responsibility for initial assessment of the patient and planning that patient's care.
They will delegate the duties as appropriate to the Senior Clinical Support Worker who will work independently or alongside trained staff to deliver all aspects of clinical care, within clearly defined limits, and attainment of defined competence.
Main duties of the job
You will support qualified staff in delivering clinical care of patients and maintaining the smooth running of the department. As part of the Trust's commitment to patient flow and improving access to care you will be involved in the direct admission and discharge of patients from the theatre department.
This is a generic job description for all Trust Clinical Paediatric Services.
Extra training may need to be undertaken on local competencies related to tasks according to clinical speciality.
Responsibility for contributing to an effective Health Care Service. Working with and providing support to all other professionals within the Multi-disciplinary Team by undertaking delegated tasks, with supervision available if needed, for which the registered professional remains accountable.
Assisting patients to maintain their activities of daily living. Carry out basic administrative and house-keeping duties as delegated.

2. JOB PURPOSE

3. MAIN DUTIES/RESPONSIBILITIES

The Senior Clinical Support Worker will be expected to:
- Work alongside or independently with qualified nurses or practitioners, including medical staff or the multi-disciplinary team in the delivery of individualised care to children, young people and their families.

- Demonstrate own duties to new or less experienced staff members and when delegated carry out training and assessments of their level of competence.
- Ensure that no action or omission on your part is detrimental to the condition, safety, wellbeing and interest of the patients and families.
Attending to the hygiene needs of the patient with privacy and dignity, focusing on the needs of the child young person, family /carer and meeting their cultural and spiritual requirements by:
- Performing nasal or oral suction when clinically indicated or obtaining a naso-pharyngeal aspirate.
- Undertaking Tracheostomy tube changes as required.
- Assisting patients with personal hygiene, encouraging self-care where possible. Observe and record pressure area care status.
- Assisting patient with toileting needs and recording output on appropriate documentation.
- Carrying out urinary catheterisation when required.
- Supporting the multidisciplinary team in promoting the care of the patients skin through the use of correct moving and handling techniques, equipment and use of pressure relieving aids; reporting any changes in skin condition.
- Attending to additional hygiene needs such as gastrostomy site and tube care, tracheostomy care including suction and tape changes,
- Removal and replacement of simple wound dressing and removal of an intravenous cannula under instruction.
- Ensuring the patients environment is clean and safe to use throughout their admission.
- Cleaning and making a bed and preparing a bed space after discharge of patient.
Attending to the nutritional needs of patients by:
- Recognising different feeding regimes and being able to administer them as required
- Supporting nutrition when oral feeding is not possible by inserting a nasogastric tube (NG tube) when instructed to do so by registered practitioner and testing the correct positioning of the tube before each use.
- Giving feeds via various feeding routes ie a naso-gastric (NG) tube, gastrostomy (PEG) naso-jejunal (NJ) or percutaneous endoscopic jejunal (PEJ) tube.
- Assisting patients with provision of food and drink and with eating and drinking, including infant bottle feeding and use aids as required according to the childs and young persons age and ability.
- Completing Breastfeeding assessments to enable effective support to be given.
- Facilitating mothers to breast feed or advise on sustaining breastfeeding. Encourage breastfeeding mothers to access appropriate nutrition during hospital stay.
- Recording nutritional and hydration intake on the appropriate documentation.
Assess the patients condition and contribute to the delivery of care by:
- Taking/measuring and recording physiological observations on PEWS chart and reporting findings to the qualified practitioner.
- Undertaking venepuncture and cannulation where clinically indicated.
- Obtaining and processing capillary blood gas specimens
- Undertaking agreed clinical activities relevant to the post & clinical area (in accordance with Trust policy) and recording actions/ findings on the appropriate documentation. This may include blood glucose monitoring or obtaining specimens of bodily fluids.
- Contributing to recording of nursing evaluation in patient documentation in line with Trust guidelines.
- Instigating emergency procedures if patients condition deteriorates.
Maintaining an environment for the delivery of safe and effective patient care by:
- Escorting patients between departments, eg X ray or Theatre including the intra-hospital transfer of a patient who is sedated.
Assisting in the support of families in distressing situations, ie. a deteriorating child,
Patient Care duties may include:
- conflict or bereavement.
- Contributing to maintaining a clean and appropriate working environment for clinical procedures and assist a health care professional with procedures as required.
- Contributing to the effective running of the department by answering and relaying telephone messages accordingly, attending to the general tidiness and cleanliness of the ward/department as per Trust policies, and undertake errands as required.
- In the absence of the ward clerk to tidy case notes and file documentation correctly,
- Assisting in department equipment checks as delegated including oxygen, suction, resus trolley, fridges and kitchens etc ensuring all equipment is in good working order and ready for use.
- Maintaining tidy linen storage and efficient, safe removal of dirty linen.
- Being aware of the importance of play for children and young people and working with the Play team to ensure that all patients are involved with clean play equipment and activities as appropriate.
- Topping up clinical areas with stock from store room, order supplies under direction of Ward Manager.
- Helping parents, relatives and friends to communicate their needs eg ; to overcome potential language barriers and cultural differences.
- Supporting patients and families to make choices about their care and ensure that those choices are respected.
