Transition Nurse - Trust-wide Medicine Care Group
An exciting development opportunity has arisen for an experienced RSCN Nurse to undertake a secondment to develop and drive forward quality focused care for young people who require transitional care across the specialities within Sheffield Children's Hospital.
We are looking for an experienced nurse with knowledge of transition processes for young people transitioning into adult services. Excellent communication & leadership skills are required, along with the ability to work independently and within the multi-disciplinary services. You should also possess a real passion and interest in adolescent health and transitional care.
The post holder will be involved in transition planning across the Trust which you will be able to drive forward and share your learning expertise. We are a small team consisting of a Lead Nurse, Consultant Lead, Management lead, Transition Co-ordinator and Team Admin.
This a 7.5 hour post, (1 day per week). Part of the role will require the post holder to attend some key meetings as and when required both local and within the Yorkshire and Humber Network (day to be negotiated). There will also be an opportunity to undertake audit work during this secondment.
Main duties of the job
- The coordination and progression of the transition database which identifies the population of young people requiring transition plans for all specialties. The database should allow tracking of the young person through their transitional programme. (Monitoring of DNA rates should be closely monitored to ensure a young person does not get lost in transition).
- Ensure trust transition related policies and pathways are up to date and implement national guidance as appropriate.
- Coordinate the benchmarking of all specialities with the Department of Health's "You're Welcome" young person quality standards so that each speciality may develop suitable action plans to improve the quality of service provision.
- Develop suitable transition training packages for various staff groups with the Trust.
- Ensure all specialities have a suitably published transitional pathways and operational guidelines which are developed jointly with adult services. (New development should include young people themselves in the development of these pathways).
- Ensure every speciality has a named health care professional responsible for their transitional arrangements.
- Develop young people's forums that will seek the views of people using the transitional service and inform future service development.
